Brewers �Two for Won� Promotion
Double flight miles when you fly on any day the Brewers win a regular season game.

Register for the �Two for Won� promotion and get double flight miles when you fly on any day that the Brewers win a regular season game (triple miles for post-season wins). Regular season games are from April 7 through October 4, 2009.

Also, if a Brewers player hits a home run that strikes the Midwest Airlines bullpen sign at Miller Park during regulation play, all �Two for Won�-registered members will be eligible for a drawing to use the Midwest Airlines Suite at Miller Park on Sept. 24, 2009. If the qualifying home run occurs after Sept 24, alternate arrangements will be made. Only one winner will be selected for the 2009 season.

* Members must fly on regularly scheduled Midwest operated flights, excluding codeshare flights. Tickets that do not earn Midwest Miles are not eligible.

�Two for Won� Bonus Miles Offer Terms and Conditions

> Promotion Description:
Registered Midwest Miles members will earn double flight miles for any qualifying flight flown on a day the Brewers win a regular season game. Suspended, resumed and postponed games will be counted as the day the game is considered complete and a winner declared. A qualifying flight includes travel on Midwest Airlines/Midwest Connect-operated flights (excluding codeshare flights) April 7 through October 4, 2009 on published fares.

> Registration:
You must be a member of the Midwest Miles program and register for this promotion to participate. Midwest Miles members may register for the promotion at midwestmiles.com/24WON or by phone (800-314-7125, option 3, option 2) using promo code 24WON. If you are not a Midwest Miles member, first go to midwestmiles.com to enroll, then register for the promotion. Only one registration is required.

> Eligibility:
Registrations on file at the end of the month will qualify for participation in the promotion for Brewer games and qualifying flights in the current and subsequent months. Retroactive credit will not be given. The member�s Midwest Miles account number must be in his/her reservation at the time of travel and Midwest Miles earned for that flight to qualify for this promotion.

> Award Fulfillment:
Bonus miles from this promotion will be posted to the Midwest Miles member�s accounts monthly within 10 business days of the end of each month. Individuals may not earn bonus miles in multiple accounts in this promotion. Duplicate account activity from this promotion will not be combined under a single account.

> Other terms and conditions:
All Midwest Miles program rules apply. Midwest Airlines is the final authority on the interpretation of the Midwest Miles program and promotion rules.

Suite Spot Home Run Contest Terms and Conditions

> Promotion Description:
The first time a Milwaukee Brewer player hits a home run that strikes the Midwest Airlines sign in the Brewers bullpen at Miller Park on a fly during a regular season game (�Qualifying Home Run�), all Midwest Miles members who are currently registered for the �Two for Won� promotion (described above) will be entered into a drawing for the use of the Midwest Airlines Suite at Miller Park on Sept. 24, 2009.

> Eligibility:
NO PURCHASE NECESSARY TO ENTER OR WIN. Contest is open to Midwest Miles members who are registered in the �Two for Won� promotion and are legal residents of the United States, 18 years old or older. (See enrollment and registration information above.) Midwest Miles membership and promotion registration is free and a qualifying flight is not required to be eligible to win. Offer void where prohibited. Employees of Midwest Airlines and their respective agencies, subsidiaries and parent companies, or their immediate families are not eligible to participate or win. All applicable federal, state and local laws and regulations apply. Members must be registered at the time of the Qualifying Home Run to be eligible. Limited to one entry per person. Entry constitutes permission to use winners' names, addresses, voice and/or likenesses for advertising or promotional purposes in all forms of media worldwide without further compensation or payment, except where prohibited by law.

> Award Fulfillment:
Drawing will be held and winner notified by phone, postal mail or e-mail within 10 days of the first Qualifying Home Run. One random Midwest Miles member, from all �Two for Won� promotion registration entries, will be selected to use the Midwest Airlines Suite at Miller Park on September 24, 2009 (�Prize�). Winner will receive 20 suite game tickets and six parking passes. A Prize will only be awarded to a single winner for the first Qualifying Home Run of the regular season.

> Terms and Awarding of Prizes:
Midwest Airlines reserves the right to end this promotion or vary the Prize at any time without prior notice.

This contest may be terminated at any point without prior notification. Rules shall be governed and enforced pursuant to Wisconsin law, excluding choice of law provisions.
